MedWatch - The FDA Safety Information and Adverse Event Reporting
Program
Annals of Internal Medicine published an article reporting three
patients who experienced serious liver toxicity following
administration of Ketek (telithromycin).
These cases were also reported to FDA MedWatch.
Telithromycin is marketed and used extensively in many other
countries, including countries in Europe and Japan. While it is
difficult to determine the actual frequency of adverse events from
voluntary reporting systems such as the MedWatch program, the FDA is
continuing to evaluate the issue of liver problems in association
with use of telithromycin in order to determine if labeling changes
or other actions are warranted. As a part of this, FDA is continuing
to work to understand better the frequency of liver-related adverse
events reported for approved antibiotics, including telithromycin.

An in Vitro Study of the Activity of Telithromycin against Mobile and Cystic Forms of Borrelia afzelii.
Brorson O, Brorson SH.
Dept. of Microbiology, Vestfold Sentralsykehus, Tonsberg, Norway.
In this study the new ketolide telithromycin was tested in vitro against motile and cystic forms of Borrelia afzelii, one of the species of Borrelia burgdorferi sensu lato. Acridine orange staining, dark field microscopy, and transmission electron microscopy were the techniques used to study the influence of telithromycin on the bacteria. The activity was unexpectedly high, 0.0003 microg/ml < MBC </= 0.0006 microg/ml for the mobile forms after 7 days of incubation at 34 degrees C. MIC was < 0.00015 microg/ml. It is likely that the agent works bacterostatically and kills in a time-dependent and concentrationindependent way, by binding tightly to the ribosomes. The agent was not able to prevent cyst formation, and the cysts were not affect ed at an in vivo achievable concentration.Electron microscopy also supports the hypothesis of telithromycin being an effective agent against the mobile bacteria
Infection. 2006 Feb;34(1):26-8.

Comparison of in vitro activities of ketolides, macrolides, and an azalide against the spirochete Borrelia burgdorferi.

Two ketolides, three macrolides, and one azalide were tested in vitro against 17 isolates of the B. burgdorferi s.l. complex. As measured in micrograms per milliliter, activity was highest for cethromycin (MIC at which 90% of the tested isolates were inhibited [MIC(90)], 0.0019 micro g/ml) and telithromycin (MIC(90), 0.0078 micro g/ml). Electron-microscope analysis and time-kill studies also supported enhanced effectiveness of both ketolides.

ketek
Merci,
